SMGF asks that you include a pedigree chart with your DNA sample. Your pedigree chart should show ancestors
on each family line who were born before 1850 or born outside of the United States. You may download a blank
pedigree chart as a pdf file.
Please carefully write the full names, dates of birth, and places of birth for yourself and all ancestors
n the form. If you only have partial data for an ancestor or family line, please include what you know and indicate
that futher information is unknown.

as a guide for filling out your pedigree chart. We welcome additional data such as marriage and death
dates in your genealogy. We also appreciate information on earlier generations in your family tree.
Our genealogy staff will convert your pedigree to the SMGF format and verify that basic information is correct.
When proven genealogical data is available, we may extend your pedigree further back in time.
You may submit a print-out of your genealogy as long as it contains the same information as what is on the form.
You may also submit your family tree as a GEDCOM or PAF file on diskette or CD in place of the pedigree file. Your
name should be listed as the primary person in the pedigree. Unfortunately, we cannot accept data in MAC format.
GEDCOM is a widespread genealogy file format. All major genealogy programs (such as Family Tree Maker) can convert
from proprietary format to GEDCOM. Please see the Help section of your program for more information.
PAF (Personal Ancestral File) is a free genealogy program distributed by FamilySearch.org. Look for the link to
Free Family History Software on www.familysearch.org.

of language and tribal/clan affiliation if applicable. This information will be used for population studies, and will not appear
