Fill out the sections shown below in yellow on page 3.
A parent or legal guardian should sign and date the section shown in green.
Please ask an adult witness to sign the section shown below in orange.
(The witness does not need to be a notary public or other official.)

You do not need to fill out the remaining sections on page 3.
You do not need to fill out the bottom of page 4 unless you have read the consent form to a person who cannot read or write. In that case, please sign and date the bottom of page 4.

If you are age 7 to 12, please sign and date and the section shown in yellow on the Research Subject Assent Form. The parent or legal guardian who signed on page 3 should sign and date the section shown in green.
