A parent or legal guardian should sign and date the section shown in green.
Please ask an adult witness to sign the section shown below in orange.
(The witness does not need to be a notary public or other official.)

You do not need to fill out the remaining sections on page 3.

If you are 13 to 17, please sign and date and give your age in the section shown in yellow.
The parent or legal guardian who signed on page 3 should sign and date the section shown in green.

Note: If you have read the consent form to a person who cannot read or write, please sign and date the bottom of page 4.
